Fixer and Star Rewards

    How does fixer's star reward system works? So far I don't remember getting 3 stars.
    Does it depends on the heals and damage I do within certain durations? Or Captain & ship's Lev and Infamy?
    I checked my battle log yet I couldn't come up with an answer.
